FIRST VISIT

Your First Visit to Wallaceburg Dental Centre

Your initial appointment at Wallaceburg Dental Centre will primarily be a consultation to discuss your diagnosis and potential treatment options. In some cases, treatment can be carried out on the same day as the consultation. However, if you have a complex medical history or treatment plan, an evaluation will be required, and a second appointment will be scheduled for treatment on a different day.

To assist us in providing the highest level of care, please provide the following information at the time of your consultation:

Any referral slip and X-rays, if applicable.

A list of medications you are currently taking.

If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e., antibiotics, for pre-med.)

Insurance Information

Do you have dental insurance or dental benefits? Wallaceburg Dental Centre accepts all major forms of private Dental Insurance. We also accept Government coverages such as Ontario Works (OW), Ontario Disability Support Program (ODSP), Healthy Smiles Ontario (HSO), Non-Insured Health Benefits (NIHB), and Ontario Seniors Dental Care Plan (OSDCP). If you’re unsure if we accept your Dental Benefits Plan, feel free to call us, and we’ll be sure to let you know!

We also offer Direct Insurance Billing for our patients! This means we will bill your insurance provider directly with your claim and take care of the claim processing for you. This leaves you to pay any fees associated with your plan, such as co-pays, deductibles, etc. We’re here to help answer any questions you may have about your insurance plan!
